A knob screwed into the handwheel would work, but I'm lazy and don't want to drill if I don't have to.

Consider the guy on eBay OUTED!!!!. Here is a pix of the window crank he uses and it even has the same patent number on the handle. It does take a fair amount of work to make one like he is selling, but you could do it considerably cheaper. I think the crank that I took a pix of was around $6.00.
