Hydrogen peroxide is usually a good way to clean dog accidents from carpet (test in a hidden spot first to make sure there's no bleaching). Take her out immediately after she eats, first thing in the morning and last thing at night, and any time she seems restless. Praise her lavishly while she goes in a proper place. She will learn. She's been through some rough times, but once she understands that this is really her home, the problem should go away. Best wishes.
