I include the name of the recipient, the maker, the location where it was made, the year, the name of the quilt, and perhaps the reason for its making or the story of the quilt.
Often I print this on fabric I've run thru my printer (fused to freezer paper cut the size of the printer paper) using borders from the Word program and then color the label with Pigma pens afterward.
