I would hand sew the 'candle to the center of the flower, and then probably hand sew the flower to the top of the cake. Using Velcro to attach the flower to the top sounds great and would work for an older child that wouldn't loose the candle part. My GDs are 5, and twins 3, and things that go together still don't stay together in the same place, so the candles would need to be attached.
