I made a rag quilt with cotton on the front and 2 layers of flannel. I wanted to use flannel as the innards as I wanted a fluffy, thicker ragging. No, it isn't puffy when you use the flannel vs batting but then I didn't have to cut all the batting pieces smaller than my squares. I used my Accuquilt Go with the Rag die and I just put a layer of cotton and 2 layers of flannel thru the machine at a time. This way, my squares were all assembled and ready to be sewn together. Also, it is plenty warm enough with the 2 layers of flannel!
