I use a yard stick to give me enough slack in the hanging sleeve. I sew the raw edges of the sleeve on with the binding and then finish the binding as usual. Then when I'm ready to do the hand stitching on the sleeve I take a yard stick and put it thru the sleeve and then push it up so it's just below the top edge of the binding and re-iron a new crease in the sleeve for the hand stitching line.
