I pin my sleeves to the top of the quilt back prior to binding and then stitch the binding onto the front, making sure of course you have plenty to catch with the stitching... then the top of the sleeve stitching is covered when I handstitch the binding down, then I handstitch the inside of each end of the sleeve to the backing and handstitch the bottom of the sleeve to the backing.
