Glad you will join us! No e-mail -- we connect with Lori through the PM function. Go to the first post on this thread to find the questionnaire. What I do is copy and paste the questionnaire into Word. Fill it out, save it, then copy it again and PM it to Lori -- you can find her in the first post on this thread. Just click on her name to send a PM. That way I have a saved copy of the questionnaire, and just change it up a bit each month if I'm looking for something different.
