It's 18 inches square
It's hung by a hook that they sell at Xmas time. It's a magnetic hook, and RED, but our door is metal, and the hook is hidden by the wreath. I put triangles in the back of the piece and so far just balancing it on the hook keeps it centered.
I just thought of something - I've seen little metal hooks with magnets on them that I think would work too, when I go to Home Depot or Lowes with my husband
2 reasons I don't hang a traditional wreath:
There's only a few inches between the doors (a fatter wreath would get squished), and in the past a traditional wreath, usually on a wire frame, scratched up the door.

And to the person that commented on 'perfect quilting' With this one I learned to make sure the quilting pattern is not hidden in any area by the binding. But, I have never had anyone from the quilt police come to my door and ring the doorbell, though I do suppose it's possible they came to the door, looked, and ran away laughing!
