Default Labeling commission quilt and charity quilts

I just made my first commission quilt and don't know what should go on the label. Usually I put the name of the quilt, my name, town and state, and the year. If it's a gift I put who it was made for, such as grandchild or whoever with "Made with love for Anastasia N...." This quilt is to be a gift for my friend's new great-grandchild. I don't feel my name should be prominent the way it is when I give a gift. I plan to ask my friend what she would like to call it, giving her some suggestions as this is a new idea for her. What do you think?

Also, I've got a couple of comfort quilts going for a guild project like Project Linus, but I think they will go directly to a specific hospital or other group. In the past I've used "Made for you will love by Grammy Lynn, [then my town, state, and year.]" I thought it would be comforting to someone. Is there a procedure for that kind of label too?
