Yes, I label my quilts. I chart the wording on graph paper and back stitch (an outline stitch for cross stitch) the information on 18-count, antique white Aida cloth using embroidery floss in a color from the quilt, making sure it is a dark enough color to read.

The label should be as big as it needs to be in order to hold the information you want to include.

The minimum information to include would be the name of the quilt, the recipient's name, your relationship to the recipient, the year(s) the quilt was made and where it was made.

It's good to have that information because life often gets so busy, you do tend to forget, and years from now, heirs to your quilts will be grateful for it.
