This is what I do as well. Try to get the knot as close to the end of my current line of stitching as possible, do as above, then pull up about an inch away. Kind of scrunch up the quilt top a bit and clip the thread. At that point, the end of the thread stays buried in the sandwich when you flatten it back out again.
