I did find the same quilt here
http://amishloft.com/quilts/bed/quilt_q127.html with a few words about the maker, though it's done trapunto style and not appliqued. Amish Country Lanes has many similar style quilts. I believe these ladies make their living by creating these beauties, all hand sewn, so I don't imagine they would sell patterns. But they will make them to order. Otherwise it should be a fairly simple task of recreating something similar. I'm sure one is on my 'to-do' list one day.
