A very long time ago, I saw a little hint on tangling thread. When you are pulling thread from a spool for hand sewing, take the cut end and place it on your shoulder. Pull the length of thread you want and cut it. The end that is on your shoulder is the end to put the knot. Voila! The thread won't tangle as you sew.
I've done that for years and it works. I too use much longer than the recommended 18" cuts.
Too much start and stop that way.
