It sure looks like the Log Cabin Tumbling Blocks by Flavin Glover to me. If you go to her home page and click on order form on right hand side of page, she has a primer and template for that same name for $6 (very last item on order form) The pictures are exactly what you see in OP's photo although in a different color way. $6 is very reasonable in today's market.
