This may have already been discussed, but this is what I was taught in a class I took on landscape quilting. On the parts we wanted to stand out (or be poofy), we cut the same pattern piece out of the batting (only a smidge smaller) then adhered with spray to back of the fabric piece. Stitched it to the background piece, etc,. Then a piece of batting was used to back the whole thing. I'll bet this is mud clear. IOW, the 'poofy" parts will have two layers of batting behind them.Then the whole quilt is quilted as usual.
