I had one machine with a lay-in slot on the take-up lever that the thread kept hopping out of the slot. I took a bristle off of an old brush (fairly stiff bristles) and glued one end of it to the take-up lever pointing so that the thread could pass by it going in, but not coming out. It worked, and was barely noticeable.
