I make them for an assisted living near me that has a dementia unit. They've asked me not to use zippers because fragile skin could get caught in them. I would not use buttons either, they could be swallowed. Picture below of one of mine. I triple stitch each seam because I expect these get treated rough in the laundry. They tell me they are holding up well. I have lace attached on one side only to "finger", a cord which is knotted on the end and can go in a pocket, corduroy to touch, more lace which can be moved from side to side, textured ribbon securely sewn and a plastic ring on a cord. The entire quilt is about 18" square. I didn't put batting so it would be cool in the summer. The back has a fleece like fabric. I'm very careful about what I attach because I'd hate for someone to swallow something they should not. For my next batch they've requested I put some fabric with pictures of recognizable items like animals or foods. Can do!!
