I create mine on the computer, then print them out on fabric made for ink jet printer, cut them out and 'frame' them into the back of the quilt. I'd like to find an easier way to do it, but til then it's been working.
I include the quilt name, who it was made for, and the date as a minimum of information.
I found a web site that has great art work to be used for scrap booking but I use it for binder cover inserts and quilt labels. If I can't get it into this post I'll send it in the next.
