I use Quilt Album. Its a free software program that can be found here
http://www.quiltalbum.com/ It is really simple to use and you can also add pictures of the quilt. There is a form to add name of quilt, start & finish dates, pattern, designer, batting, quilter, etc. and there is a note section.
