I wanted the same thing. About a year ago I researched and found a journal for recording quilts and quilting details just as you and other suggested. There are two pages for each quilt with lots of room to record what you need. It is from the 'Write it Down' series and is titled "Quilting: The Quilter's Journal." I also have realized that I need to update mine from reading this thread. I also like the idea of keeping records online, but I like to have a physical book to share and show people who might not be near my computer.

One hint: if you decide to use something like this book, I suggest you print out photos for the quilts on paper. I have glossy prints and my book is not staying closed very well. I think I will print the photos on paper and remove the photo prints. I will be happier with my book that way.
