Are you sure you have the belt
under the belt guard (the piece that has the bobbin winder attached)? You have to thread it under the belt guard not just place belt over the hand wheel.
https://www.youtube.com/watch?v=lXEoZIBserE talks about it at approximately the 3 minute mark. Granted not a Singer 15, but it is a 15-clone, so the treadle belt would be the same. Something else I would check would be that it is threaded through the "throw off" thingy by the underneath fly wheel.
