Have you thought of getting a curved upholstery needle to use for your thread basting? When I sandwich my quilt I use big binder clips all around the table edge so getting my hand under the fabric is impossible. When I baste (usually with pins but sometimes thread) there seems to be enough 'give' in the sandwich to be able to get the pin/needle up to the top again without having to put my hand under. But a curved needle might do the trick for you??
