When I make a quilt it is often one that I have seen and so I need to draw it onto graph paper with notes. That way I don't get lost in the middle of it. If I use a pattern, I photo copy it and write notes on it - that way if I make a mistake I can re-do my notes. Then I have a Composition Book. (the kind they make journal covers) that I glue those items into it with the note of who I made it for. Not a lot of detail, because it's just a memory book for me not something to show anyone else.
