I think it depends on area, but I think generally it is an extra service. I know, seems like they could throw it in for the cost of it all, but not everyone needs or wants the help. I think it is pretty reasonable and would be well worth it for me. Their experience is worth paying for. You can watch and learn instead of learning as you go...
I was looking at used machines, basically all set-ups are both take down/set up responsibility of the buyers. The table set up I bought was extremely heavy, it is all one (welded!) piece. Marital harmony is being disrupted that it (my used set up) is still not set up, and I'm thinking $500 sounds pretty good after more or less a year of waiting for his help. Ok, half that time I couldn't see and didn't want it set up, but that time is over now!! If this had been more of an aluminum grace frame I would have had it set up long ago but even a light weight frame is helped by having two people.
