A picnic quilt. It only needs to be lap size with plenty of themed fabrics. I'd use a disappearing nine patch.

To all the great ideas above, I will add a tutu. Just a piece of elastic her waist measurement, with tulle tied around it.

I am presently making birthday gifts for a DGS & DGD who both will be 3 at the end of the month. The Grandson is getting a nap quilt with a dinosaur panel and a bag for a coloring book that has slots on the front for the crayons. Inside the bag I will put a dinosaur coloring book and new crayons in the slots. For Granddaughter I made a nap quilt from an "Elsa & Anna" panel and also a coloring bag. The bags for both are made from fabric from their respective quilt theme. Hope they like them.

a travel pillow that holds a small quilt or fleece cover-up, a tutu, a snow white cape for dress up, a sleeping bag for favorite doll, a table cover with a tea set, maybe with a pouch to hold the tea set, 4yo's are easy to please.

My daughter (4) gets super excited about new skirts. Often if I don't have a lot of time, I'll use this tute http://www.danamadeit.com/2011/04/a-...-tutorial.html

My answer for any gift is a quilt.

Her very own pillowcase, with ruffles on the band. I made pillowcases for my two grandsons for them to use while here and then take home, and they loved them. I put their names on the band in applique. No fighting over who's who's and they really had fun with them

Fanny Packs are coming back in style, you might try one of those. Would hold lots of her small stuff.

Find a pattern for a cute quillow, you cab google and find some nice ones. Kids really like them for travel or just watching tv.
