If you make a king size handquilted quilt, believe me, you dont wanna count!!!

I don't count. I just try to make all my stitches consistantly. And try to stitch as close as possible. Probably about 6-8 stitches per inch. Am getting better. Practice makes perfect.
Use a size 8 needle.

Quilt appraisers measure the top stitches only.
