I would fuse a butterfly, ladybug or flower over the hole, then use a buttonhole stitch with embroidery floss around it.

I used Retro Clean on one and it was wonderful. It was a tulip quilt with a few tiny holes that I covered with applique butterflies in reproduction fabric.
