I have been making donation quilts. In my town they want this information
    Pieced by, quilted by
    Fabric types (cotton, etc)
    Batting type
    Washing/ Cleaning instructions

So I purchased some fabric labels - found a cute photo to download - named the quilt - then added the other information and printed the labels on my computer.

If quilts are being auctioned, I think that you may need to provide the fabric / batting information for legal reasons - at least that is what I have been told.

How 'bout amending this suggestion a bit to say:

Pieced and Quilted with love by Donna Jones in honor of (insert child's name)
    Exactly what I was thinking!

the label needs to include fiber content and care instructions
you could (if the quilt is for the auction/not for the child) put created by/quilted by date and location
if it is for the child i would include the childs name/benifit reason.

    I like the amended one.

I have made thousands of baby quilts in memory of my first granddaughter (was with us for one precious hour) and I did not put a label on because the NICU did not want anything hand sewed--not even the binding. I know I have done labels for my comfort quilts but right now I cannot even remember what I said--sorry.

    Exactly what I was thinking!

I like sushi's suggestion but maybe include a sheet with care instructions?
