The water bottle carrier is HeartOfMary's pattern; she's got a free tutorial floating around on the 'net, or the pattern with a few upgrades (like the drawstring) for sale on Etsy. I've made more than a dozen of the things.
As for the insulation, it depends. If it's going to a person who will likely use it to keep their drinks cool, then yes, I use Insulbrite. (I did on this one.) If it's going to someone who just wants a handy way to carry their bottle around--kids, usually--then no, I just use fusible fleece.
