i found some links to hexagon and diamond graph paper. you could make the size you want and paper piece it.

I thought it was graph paper too but if you look at the top left it looks like it's been bound. Does anyone agree?

Can't help with the name, but maybe you can just copy it since it's yours.
You could get a sheet of graph paper and color it it, much like the cross stitch and needle point patterns. It doesn't appear to be that complicated...no curves that I can see...squares and rectangles if I'm not mistaken. |
