That's what I would do.

Sure. No need to go as far as the borders. I'd like to point out that sometimes it is easier to hand sew the replacement block than to line it up exactly using the machine. You don't have to remove any stitches beyond the piece that needs to be replaced. Personally, I would find that easier than doing a little applique, but that's just me. Use a ladder stitch and work from the front side, and it won't show at all.

Cut out lips applique and put it on - a kiss from Gramma.

Thimblebug 6000 is right. It isn't hard at all. Go for it.

put a thought bubble above the lion so the lion is saying who made the quilt .

This is inexpensive, can be purchased at Joann's (use a coupon) and works wonders. I wouldn't undo that quilt if it were mine. The quilt is as cute as can be and an applique would not hurt it, but this should fix it similarly to the mending stuff mentioned before, but a lot easier.
Have a good day, and don't stress over all our advice. :)

I would just take the block out. I would 't be happy leaving it in and giving it away. You could also just open it enough to make that seam just a hair larger.

Cut out one of those circles from the material on the quilt and applique it over it spot. No one but you would know it wasn't suppose to be there.

I think I would just applique a matching square over it. If you are very careful when ironing it and folding, you should not even be able to see the 'repair'. IMHO
