I had that problem too. I noticed my thread was rising off the spindle. I started putting a cap in the upright spool and now it winds evenly again.

I pieced mine with both extra pieces of thirt and with cotton. I stabilized the cotton with the same fusible I used on the shirts to keep everything the same feel and thickness.
