Yep. Just once, many years ago.

That's why I watch with an eagle eye as they lay out and measure my fabric. If it doesn't look right to me, I stop the clerk before they cut and ask (politely) for them to re-measure. I also keep an eye on the fabric for possible flaws and once the fabric has been marked for cutting, ask the clerk to turn it over so I can check the underneath side for flaws as well.

So far, every time I've felt that the length looked short, re-measuring proved me right.
