I work with groups that auction quilts for charity and they usually say "Machine pieced and quilted by: " Or machine pieced and hand quilted by: as the case may be. It is also good to add your city, state and date, as it is nice to know where the quilter lives.

I use "handcrafted original for-------" if I make only one, if it is a pattern of my design I add "designed and handcrafted". If it is someone else's pattern I add that information. My husband is a woodworker and after a discussion this is what we came up with to encompass all the work- hand and machine aided.
