Your idea will work fine. I once made a T-shirt quilt for a neice, and printed out several pictures on printable fabric blocks purchased just for that reason. Before I gave it to her I washed it, following the directions to the "T", but ALL of the pictures faded out - they looked like ghost pictures. I reprinted them, cut them a little larger than the original pictures and sewed them on, using decorative stitches and/or rick-rack. Worked just fine!! Good luck.

LOLOL....Reminds me of my Junior High Home Ec. teacher from many many years ago. She was a rather petite, foreign speaking lady and everytime she would tell us to get out our 'cookie shits' (cookie sheets), the whole class would be in an uproar. I don't know if she ever figured out why we laughted so hard.

I think the 'typo' in your post may have been a Freudian Slip...least-wise it would have been for me! I would suggest you make a pillow case out of the extra shirt, to go with the quilt and then it is less of a problem.It is always non - sewers who think it will be easy to change stuff.

Haven't read this entire thread but can you work some of the shirt into the borders? Might be a way to add any others that were accidentally omitted. How about a tote or can the quilt be fashioned into a quillow with the added shirt on that?
