Don't use the washing machine to try and clean up all the loose threads before sandwiching and quilting. The washer tears apart the stitching and frays the pieces. I ended up with so many holes I threw the top away. Now, I make sure to cut all the tails as I go.

I agree with this one. And wanted to add: don't cut your borders at the beginning of a project. And especially, don't randomly decide that the pattern you spent hours & hours creating suddenly has bad math, cut your fabric 10" shorter & then get the thing made only to find you actually were supposed to have it 10" longer.

Make sure your backing is straight with the top when making your sandwich. And then check it again. And yet again. You can't make certain of this alignment too many times.....because when you've quilted it, and find it's just slightly crooked, you will never be able to forget it ....or forgive yourself.
