I just ran across this -- http://quiltwoman.com/june-wedding-quilt-pattern.aspx -- which I think is really pretty.
Are your son and fiance traditional or non in terms of style? Do you know what colors they like to decorate with (bright vs. soft, etc.)?
The first thing that leapt to my mind was a reverse double wedding ring -- the rings plain ivory or light gold fabric, the background apple print fabrics. Or you could do an assortment of squares -- alternating apples (using snowball blocks) and hearts, for example.

Oh my goodness! I went to Pat Sloan's website..and LOOK what is the in the middle of that quilt? The apple block! Maybe she knew!

I would suggest an appliqued apple tree in the center and use the apples to either embroider or use Micron pen to put family names on. Surround it with some favorite blocks of your choice and a few borders.

Was just online today and found this really cute embroidered apple tree and it is a free pattern. It's called Under the Apple Tree and it is by Robyn of Daisy Quilts. Would be really nice in center of quilt.
