I have used fine point sharpies. I iron a piece of muslin onto freezer paper, then write what I want with a frixion pen (just in case I want to change something, a hot iron will erase it), then I go over the letters with a sharpie.

Micron Pigma pens. For fancy labels I hand embroider (I only have plain block letter embroidery capability on my machine)

Have used Micron Pigma pens, but usually do them on my embroidery machine.

Pigma pens, and lately I've been writing the info directly onto the quilt as inconspicuously as possible. But if I were making a "show quilt" I wouldn't mark up my quilt like that.

I go look at the pens in the scrapbook department. If it's archival for good papers it's archival for fabric too. They come in a good variety of colors and tip styles.
