What a wonderful gift!
One of my favorite pictures of Tasha, a Shiba I had prior to my current two, is with her best friend, a Cocker named Ginger. The two of them sitting together is a happy memory of how much they loved being together to enjoy each other's company. Perhaps a photo of her two dogs together would be also, knowing how much they meant to each other too. Someday, both will be gone and then the photo becomes a bittersweet memory of a time in her life and her two best furry friends.
The date you made the quilt (i.e.: 2011) would be all I'd recommend. Plus a title for the quilt, such as one of the poems/sayings mentioned in other responses?
