On the commission quilt, I don't see why a small label with the quilt title, your name, town, state, and year would not be appropriate. It would be no different than the artist naming and signing any other artwork created on commission. That information is important for the provenance of the quilt.
Offer to make an additional, more prominant if you wish, gift label with your friend's sentiments to her great-grandchild.
