See if you can find anything from Nancy Zieman (Sewing With Nancy on PBS). Her original shows (30 yrs ago) were about garment making, but she has now included quilting, and combines both.

The method I have used is to make pieces of quilted fabric (strip pieced, various blocks, etc.) slightly larger than than each pattern piece, then lay the pattern on the quilted fabric and cut the pieces out. Then sew the pieces together. I then make a lining to hide the seams. One method I haven't tried, but have seen is to cut the pattern using the next larger size, and sew the pieces together like you would a rag quilt for a different look.
