we have Plan F with State Farm and it pays the balance of Medicare approved costs, deductible, we're out nothing but monthly cost - and my husband's had 3 major surgeries plus - only thing it doesn't cover is prescriptions, good anywhere, no networks, heartily recommend State Farm
