Default ribbons & Bows quilt pattern

I am in desperate help I am making this quilt the pattern is in quilt magazine #104 Oct/Nov 2009
I am a procrastanator so just making this quilt now. It states there is a pullout but mine is missing, well I am having trouble in the assembly of the quilt I am hoping someone on this board can help me. If you happen to have this magazine and it has the pullout maybe we could get together. Or if someone could just look at it and explain the assembly to me it would be appreciated.

Sorry, I don't have the pattern you need. Perhaps if you posted pictures of what you've done some of these great quilters could offer some advice.

This is a Jackie Robinson pattern. If no one has the pull out to share you may have to buy the pattern.

The main block looks like a 'jack in the box' block, alternated with a 9 patch with the center patches being very narrow. It looks like blocks with sashing, but I think the sashing is actually in the middle of the blocks.
Look online for instructions 'jack in the box' block. Here's one from quilter's cache, but it has the 'arm' blocks oriented a bit different:
    http://www.quilterscache.com/J/JackintheBoxBlock.html

There's a really good one from MSU in PDF format. IF you google "jack in the box quilt block" it should come up. It won't let me copy the link.

I've been wanting to make this block for a while, so I had researched it before. All the online ones have that weird rectangle with triangles on the ends. I have a book with that pattern and it uses all HSTs for the arms. That's what I was planning on doing. I think it would be easier.
All the patterns by Jackie Robinson that I've seen use fabric of her design. The zigzag look of the sashing is plaid fabric cut on the diagonal of the plaid.
    Don't know if this helps...

Can you get a close up of the pattern sheet? it does look like the Jack in the box pattern from the pic.
