I would just put my name (or initials), year made, and if you like, city and state. That way you free up the co-worker to recycle the quilt when the baby no longer needs it. The co-worker is going to know that it came from your daughter, and more than likely, that you made it.

You are correct. Some organizations want the donation of a quilt to be anonymous but others such as QOV want your name on the quilt.
