I've had to stop and start many times during quilting. Do I trim those little starter and ending threads off flush to the fabric or do I tuck them into the quilt somehow?

I leave mine a little less than a 1/4" long, if I cut them up to the seam, sometimes the first stitch will come loose.

I always trim mine as I go. It makes it easier than finishing the top, then cutting the threads.

I leave little tails too. They usually disappear into the quilt.

I always chain piece, starting and ending with a scrap of fabric and as I clip my pieces apart, it leaves about 1/8 to 1/4" of a tail. Usually, I keep some pieces to another quilt near the sewing machine and start and end my chain with those. Right now, it's triangles.

I trim as I go then trim again after I finish the top.

I think you are actually talking about what to do with thread tails while quilting your sandwich. If so take a look at this video. This is how I go it and actually I pull my bobbins to the top while quilting and then once the whole thing is quilted I take the time to do this! As long as you pull the bobbin thread up and keep both top and bobbin tails out of the way of your quilting then it doesn't get caught up! I do it for begining and ending tails!
