Right off the top of my head:
Booksandbooksandbooks - Books are tools - can't quilt or design without them
Design Board (mine is 96"x96", wall-mounted)
Iron - regular sized steam Iron
Iron - travel steam iron (classes)
Ironing Board (floor and desk models)
Mats - Rotary Cutting Mat (all sizes)
Mats - totable Cutting Board/pressing board all-in-one (classes)
Needles - hand - several types and sizes depending on what I'm doing
Needles - sewing machine - Schmetz Universal Needles sizes 10 & 12 mostly
Paper - precut templates for English paper piecing (hexagons, diamonds, thimbles)
Paper - printer loadable for duplicating foundation piecing templates en masse
Paper - Thangles - all sizes of half-square and quarter-square triangles and flying geese,
Pincushion & Thread catcher (all-in-one unit)
Presser Feet - I have a veritable arsenal of presser feet
Pressing Ham
Rotary Cutters (all sizes)
Ruler - Add a Quarter Inch 6" and 12" sizes
Ruler - Add an Eighth Inch
Rulers - O'Lipfa Rotary Cutting Rulers (all sizes - MUST have the lip for stability)
Rulers - Square-Up Rulers (all sizes)
Scissors - Curved Snip-Its (I have 1 at my sewing table and 1 at the ironing board)
Spray bottle loaded with distilled water (ironing board)
Spray Starch for flannel quilts and bias-cut quilt block pieces

I have most things mentioned but really I couldn't sew without my bendy bright light (best $50 investment ever!) and my 1/4 inch foot. And if sewing curves on drunkards patch, 1/4 inch double sided tape that washes out is really useful. I probably wouldn't have tried curves without learning that technique.

Rotary cutter, mat, seam ripper, purple thang, rullers, pencils, blue painters tape, gingers, snipps, post it notes, singer 301, bernina 640, computer, iphone/ipod( got to have music). I think that all of these are the most important.
