I like this answer best, although I've never seen the thing before. I'd love to have one for my scarves. I googled "scarf hanger," and most of them have large holes to thread the scarf through. I think that would cause wrinkles. This one, if you didn't mash it too much between other clothes, would let the scarves hang without getting creases.

Do the metal clips slide down on the metal "arms"? if so, maybe it was, (or could be) used to hold finished blocks or strips/pieces of fabric for a specific project.

looks like a belt holder , looks like it could hang onto the belt buckle.

It looks like it would crease a tie.

Given the ball ends and the slide down metal clip I would venture it is for hanging silk scarves so the pinch point of a normal clip won't damage them.

I think the scarf holder is close. But what about holding skiens or loose thread for crewel embroidery. I think I have seen something like it in yarn stores.

Definitely not a dental shade guide. They come in different shades. Those all look the same shade.
They're on little pieces of plastic so they can be individually held up to the pt. mouth for shade selection.
