It is Simplicity #4993. Gotta love it. It also has a kitten and a bear in the pattern. You use the same body pieces for each animal and just change out the ears and noses.

I just love your puppy quilt. Sure gives me thought of trying to make one for my little girl.

She is the one in the picture here. Her name is Bella.

Your Bella is adorable. I'm sure she would love the quilt too. It might be a little big but, my Chihuahuas love the fleece blanket I bought for them (before I started quilting about 2 months ago). It is full size.
