Default Memory quilt label question

I made this quilt from a coworkers hubby's dress shirts. Her hubby of 45 years passed away at the age of 69. Does anyone have a nice quilt label idea? Do I put his full name? Should my name also go on it? DOB/DOD dates?

I have made something else years ago with this saying on it...."Because someone we love is in heaven, there is a little bit of heaven in our home" I want to have embroidered labels and thought it might be too long. Your thoughts will be helpful!!!

Great job on the quilt. I'm sure she will adore it.

I made quilted stars for my Mother and all of my 5 siblings in memory of my Dad. They were made out of his clothes. Following below is the link to the star which has a pic of the label I made. I created it on my computer and printed it on fabric. Hoping that my quilted creations will be passed to other generations, I make a concerted effort to label everything I make. My quilts have much more information.

I think it would be acceptable to place your name as the designer/quilter on the label. Good luck.

What a lovely top and tribute. I have only made one memory quilt from my BIL's shirts for my SIL. I used a muslin heart for the label,and hand-wrote the info: In Loving Memory of -------, birthday and date of death. I did not include my name. This one was not about me.

The design of the quilt made me think of hugs and kisses --- circles and "x"s. What a great quilt.

If you or your friend put his after shave on the quilt it will also smell like him.i would just put in loving memory of .......dob.... Dod. You could if you wished put your name on the binding. Made by.......
